The thesis analyzes community-based medicine in Lazio during a period of profound transformation in local services linked to the syndemic crisis and the “reform” promoted by the Pnrr (Mission 6 C1) and Ministerial Decree 77/2022. Through a social geography approach, a transfeminist perspective on the ethics of care, and the methodology of Participatory Action Research, the study analyzes territorial reorganization of social and health services policies and proposes interventions based on a collection of best practices identified in the field within the Lazio Region. The analysis of three case studies—Sportello Mammut, Ambulatorio Popolare Roma Est, and Progetto Sentieri Metropolitani—offers a broad panorama of operational practices to be replicated within the substantial reform of post-pandemic territorial healthcare services. These experiences are capable of envisioning alternative models of care and community-based medicine, giving substance to the community participation promoted by Ministerial Decree 77/2022, Mission 6 C1 and regional guidelines. Through a combination of qualitative techniques (in-depth interviews, participant observation, co-research, and focus groups) and quantitative techniques (GIS spatial analysis), the thesis proposes a new approach to analyzing public social and health policies, “the Geography of care for Health services” to address the gaps and inequalities in public services and support a transformation of the current paradigm of care.
La tesi analizza la medicina di prossimità nel Lazio, in una fase di profonda trasformazione dei servizi territoriali legata alla crisi sindemica e alla “riforma” promossa dal Pnrr (Missione 6 C1) e dal d.m. 77/2022. Attraverso un approccio di Geografia sociale, la prospettiva transfemminista dell’etica della cura e la metodologia della Participatory Action Research, la ricerca analizza le politiche di riorganizzazione territoriale dei servizi sociosanitari e avanza proposte di intervento mediante una raccolta di buone pratiche indagate sul terreno nella Regione Lazio. L’analisi di tre casi studio — Sportello Mammut, Ambulatorio Popolare Roma Est e Progetto Sentieri Metropolitani — offre un ampio panorama di pratiche operative da replicare all’interno della sostanziale riforma dei servizi territoriali post-sindemica. Tali esperienze sono capaci di prefigurare modelli alternativi di cura e di medicina di prossimità, dando sostanza alla partecipazione comunitaria prevista nel d.m. 77/2022, nella Missione 6 C1 e nelle linee guida regionali. Mediante la commistione di tecniche qualitative (interviste in profondità, osservazione partecipante, conricerca e focus groups) e quantitative (analisi spaziale GIS), la tesi propone un nuovo approccio di analisi delle politiche pubbliche sociosanitarie, “la Geografia della cura per la sanità”, per arginare le lacune e le disuguaglianze del servizio pubblico e sostenere una trasformazione dell’attuale paradigma di cura.
