This study describes the development and validation of a protocol for applying single-cell RNA sequencing (scRNA-seq) to bronchoalveolar lavage (BAL) samples from patients who underwent allogeneic haematopoietic stem cell transplantation (HSCT), with the aim of assessing technical feasibility and sample quality. A complete workflow was established—from BAL collection under standard clinical conditions to cryopreservation, thawing, and cell viability assessment—optimizing each step to minimize cell loss and preserve transcriptomic integrity. The protocol was initially tested on a control sample and subsequently applied to a patient affected by chronic pulmonary graft-versus-host disease (PGvHD). The procedure yielded high-quality data from 7,075 single cells, identifying eight distinct transcriptomic clusters comprising immune and epithelial populations. These results confirm the feasibility of scRNA-seq on human BAL samples, even when collected in limited volumes and under complex clinical conditions, paving the way for systematic investigations of the post-transplant pulmonary microenvironment. This work represents the first step toward a standardized protocol for single-cell transcriptomic analysis of BAL in the context of transplantation and related respiratory complications.
Lo studio descrive la messa a punto e la validazione di un protocollo per l’applicazione della single-cell RNA sequencing (scRNA-seq) su campioni di lavaggio broncoalveolare (BAL) in pazienti sottoposti a trapianto allogenico di cellule staminali ematopoietiche (HSCT), con l’obiettivo di esplorare la fattibilità tecnica e la qualità del materiale ottenuto. È stato sviluppato un percorso completo, dalla raccolta del BAL in condizioni cliniche standard alla crioconservazione, scongelamento e analisi di vitalità cellulare, ottimizzando ogni passaggio per minimizzare la perdita di cellule e preservarne l’integrità trascrittomica. Il protocollo è stato testato inizialmente su un campione di controllo e successivamente applicato a un paziente affetto da malattia cronica del trapianto contro l’ospite polmonare (PGvHD). La procedura ha consentito l’ottenimento di dati di elevata qualità da 7.075 singole cellule, con identificazione di otto cluster trascrittomici distinti comprendenti popolazioni immunitarie e epiteliali. I risultati confermano la fattibilità della scRNA-seq su BAL umano anche in campioni di volume limitato e in contesti clinici complessi, aprendo la strada a studi sistematici sul microambiente polmonare post-trapianto. Questo lavoro rappresenta il primo passo verso un protocollo standardizzato per l’analisi trascrittomica a singola cellula del BAL in ambito di trapianto e complicanze respiratorie correlate.
